You can download the questionnaire here. It is a text file (zipped) which you can open with any text editor. When you have completed it, please email it back to me as an attachment, and I will contact you afterwards.

The zip file also contains the Client Contract and Waiver that you will need to read, print out, sign and return to me by mail along with the required fee, after we have agreed to proceed with the training. If you don't have a printer please let me know either by telephone or email and I'll send a copy to you.

To unzip the file you will need a zip utility (if you don't have it already); there are many freeware and shareware ones such as PowerArchiver (free, for PC) or Aladdin Stuffit Expander (free, for the Macintosh).
